Most public health efforts focus on improving clinic appointments, but healthcare facility strategies failed to produce any significant increase. Instead, sending medical workers with vaccine vials into places where teenagers already gather removes the friction of booking doctor visits.
When immunization programs operate inside schools, staff members set up injection stations on campus grounds. This setup operates like a mobile field clinic that meets students during their normal day. Nurses deliver the shot on site and record the dose directly into student logs. For families outside school programs, reminder systems using direct messages generate the most consistent benefit for completing the series.
Researchers analyzed thirty-eight randomized clinical trials covering 339,891 participants for vaccine initiation and 109,627 for series completion. The team compared community-and-school, digital, educational, healthcare facility, and reminder interventions against standard controls. School programs increased initiation with a risk ratio of 2.16, while reminder systems produced a 1.20 ratio for initiation and 1.21 for completion.
The authors state that health agencies can achieve substantial coverage gains by relocating vaccination services to school venues. They emphasize that adequately powered trials in low-income and middle-income countries are still needed before translating these findings into public policy.
