Childhood maltreatment directly raises the risk of severe psychiatric illness in young adulthood. Researchers often assumed that shared family genetics accounted for why abused children later experience psychosis or mania. Comparing identical twins with identical DNA proves that experiencing individual abuse drives psychiatric symptoms independently of inherited genetics.
Severe physical neglect or abuse acts as an unshared environmental injury that alters mental health development. This divergence functions like two identical plants receiving different soil conditions, where one develops structural damage despite identical seeds. The exposed individual experiences higher rates of hallucinations, paranoia, mania, and self-harm during early adulthood. In contrast, peer bullying differences largely disappear once genetic factors are removed, leaving maltreatment as the primary direct environmental driver.
Researchers tracked twins from the Twins Early Development Study born in England and Wales across assessments up to age 26. The team tested 17 severe mental health outcomes across 13794 participants at age 21 and 11436 participants at age 26. Abused identical twins showed a pooled odds ratio of 1.85 for severe conditions across 14 estimable outcomes, while peer victimization retained links to only two symptoms.
These findings establish childhood maltreatment as a modifiable pathway that clinicians can target through trauma-informed care. Early interventions targeting home abuse can prevent severe psychopathology before symptoms emerge in early adulthood.
